Compensation: $95,000 - $125,000 annually based on experience, availability, and preferred schedule.

Qualifications

Responsibilities
  • Evaluate and treat patients in a home health setting with a focus on restoring mobility, strength, function, and independence.
  • Develop individualized, evidence-based treatment plans tailored to patient goals and needs.
  • Provide skilled interventions including therapeutic exercise, manual therapy, neuromuscular re-education, balance training, and functional mobility activities.
  • Assist patients in safely returning to daily activities, work responsibilities, community participation, and recreational pursuits.
  • Educate patients and caregivers regarding diagnoses, treatment progression, home exercise programs, safety recommendations, and long-term wellness strategies.
  • Complete timely and accurate documentation of evaluations, treatments, progress notes, and outcomes.
  • Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare professionals to maximize patient outcomes.
  • Maintain effective communication with patients, families, referral sources, and team members.
  • Manage schedules efficiently while maintaining high-quality, compassionate patient care.
  • Contribute to a positive clinical culture focused on excellence, teamwork, and continuous improvement.
Skills & Clinical Areas
  • Geriatrics: Experience treating balance deficits, weakness, mobility limitations, arthritis, chronic pain, fall risk, and post-surgical conditions in older adults.
  • Home Health: Ability to work independently while assessing home safety, educating caregivers, and developing functional treatment plans that promote independence.
  • Neurology: Knowledge of stroke rehabilitation, Parkinson's disease, neuropathy, vestibular disorders, gait dysfunction, balance retraining, and motor control interventions.
  • Orthopedics: Experience evaluating and treating musculoskeletal injuries, joint replacements, spine disorders, tendon injuries, fractures, and chronic pain conditions.
  • Physical & Rehabilitation Medicine: Strong skills in functional mobility training, neuromuscular re-education, strength development, endurance training, pain management, and interdisciplinary care coordination.
  • Excellent communication and patient education abilities.
  • Strong organizational, time-management, and documentation skills.
  • Ability to adapt treatment plans to meet diverse patient needs and environments.
