Klarus Home Health is seeking a Licensed Physical Therapist to join our home health team serving Albuquerque, NM and surrounding areas. You will conduct assessments, develop plans, implement treatments, and evaluate patient progress in a one-on-one home setting.

Qualifications

• Graduate of an accredited School of Physical Therapy.
• Current state license or multi-state PT Compact license.
• Minimum of two years in clinical care; home health or geriatrics preferred.
• Excellent interpersonal and communication skills.
• Reliable transportation with valid auto liability insurance.
• Ability to lift up to 50 lbs and travel locally.

Responsibilities

• Evaluate patients' physical and functional abilities to develop individualized care plans.
• Plan, implement, and modify PT treatment programs.
• Provide hands-on therapy, exercises, and techniques to improve mobility and reduce pain.
• Educate patients and families on exercises, body mechanics, and injury prevention.
• Collaborate with interdisciplinary teams for coordinated care.
• Maintain timely documentation of evaluations, progress notes, and plans.

Klarus Home Health is seeking Licensed Physical Therapists to join our team to providing PT services including assessment, treatment planning, implementation, and evaluation for our patients currently enrolled in our home health program in the Albuquerque, NM and surrounding areas.

Requirements

• Evaluate patients' physical and functional abilities to develop individualized care plans.
• Plan, implement, and modify physical therapy treatment programs based on patients' medical referrals and assessments.
• Provide hands-on therapy, exercises, and techniques to help patients improve mobility, relieve pain, and restore function.
• Educate patients and their families on therapeutic exercises, proper body mechanics, and injury prevention techniques.
• Collaborate with interdisciplinary healthcare teams to ensure coordinated and holistic patient care.
• Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans.
• Participate in ongoing professional development activities to stay abreast of current trends and best practices in physical therapy.

Experience:

• Graduate of an accredited School of Physical Therapy.
• Current State license as a Physical Therapist, or a multi-state license issued by a state member of the National Physical Therapy Compact (PT Compact), if practicing in a state member of the PT Compact.
• Minimum of two years of experience as a Physical Therapist in a clinical care setting, with experience in Home Health, geriatrics, or medical-surgical preferred.
• Excellent interpersonal and communication skills.
• Strong nursing skills as defined by generally accepted standards of practice.
• Reliable transportation with valid and current auto liability insurance.
• Ability to work flexible hours and travel locally as needed.
• Ability to lift up to 50 lbs. and perform physical tasks associated with patient care.
