Job summary

A List Therapy Group is seeking a licensed Physical Therapist (PT) to provide home health services across California as an independent contractor. You’ll evaluate patients in their homes, develop care plans, and support mobility, safety, and independence while adhering to payer requirements and physician orders.

We offer a flexible schedule, opportunity to choose coverage areas, a digital workflow, and a generous compensation package including a $95–$120 per completed visit and a $500 sign-on

Qualifications

California PT license is required.
Reliable transportation with valid driver’s license.
Professional liability insurance.
Independent contractor arrangement.

Responsibilities

Provide physical therapy services in home health settings in accordance with laws, orders, and plans of care.
Evaluate patients at home and deliver skilled PT interventions to support mobility, safety, independence, and quality of life.
Develop and update care plans aligned with physician orders and payer requirements.
Assess home environments and recommend equipment, safety strategies, or caregiver education as appropriate.
Communicate with referring agency, physician, patient, family, and other care participants as needed.
Document patient progress accurately in accordance with payer, agency, and regulatory standards.

What You’ll Do as a Physical Therapist:

Provide physical therapy services in accordance with applicable law, physician orders, payer requirements, and the authorized plan of care.
Evaluate patients in their homes and provide skilled physical therapy interventions to support mobility, safety, strength, balance, functional independence, and quality of life.
Develop, update, and implement care plans consistent with clinical judgment, physician orders, payer requirements, and applicable professional standards.
Assess home environments and recommend equipment, modifications, safety strategies, or caregiver education as clinically appropriate and legally permitted.
Communicate with the referring agency, physician, patient, family, PTA, or other authorized care participants as clinically appropriate and legally permitted.
Document patient progress accurately and efficiently in accordance with applicable payer, agency, and regulatory requirements.
