Job summary

A List Therapy Group is a therapist-owned home health therapy referral and coordination company. We are seeking a Home Health Physical Therapist (PT) in California to provide in-home PT services with a flexible schedule and contractor opportunities.

Sign-on and referral bonuses are available, and compensation is based on visit volume and coverage area. The role emphasizes autonomy, streamlined digital workflows, and supportive administrative resources to help clinicians deliver high-quality care

Qualifications

• Current California Physical Therapist / PT license.
• Reliable transportation and valid driver’s license.
• Professional liability insurance.
• Ability to operate as an independent contractor.

Responsibilities

• Provide physical therapy services in patients' homes in accordance with laws, physician orders, payer requirements, and the plan of care.
• Evaluate patients at home and deliver skilled PT interventions to improve mobility, safety, strength, balance, independence and quality of life.
• Develop and update care plans aligned with clinical judgment and payer requirements.
• Assess home environments and recommend equipment, modifications, safety strategies, or caregiver education.
• Communicate with referring agencies, physicians, patients, families, and care participants as appropriate.
• Document patient progress accurately and efficiently per payer, agency, and regulatory requirements.

What You’ll Do as a Physical Therapist:

• Provide physical therapy services in accordance with applicable law, physician orders, payer requirements, and the authorized plan of care.

• Evaluate patients in their homes and provide skilled physical therapy interventions to support mobility, safety, strength, balance, functional independence, and quality of life.

• Develop, update, and implement care plans consistent with clinical judgment, physician orders, payer requirements, and applicable professional standards.

• Assess home environments and recommend equipment, modifications, safety strategies, or caregiver education as clinically appropriate and legally permitted.

• Communicate with the referring agency, physician, patient, family, PTA, or other authorized care participants as clinically appropriate and legally permitted.

• Document patient progress accurately and efficiently in accordance with applicable payer, agency, and regulatory requirements.
