Job summary

Main Line Health Home Care is seeking a Physical Therapist to deliver in-home PT services, performing evaluations, planning care, and administering treatments to patients in their homes. You’ll help patients regain mobility and independence, guided by physician referrals, with a focus on personalized home-based plans.

The position offers tuition reimbursement up to $6,000 per year for full- or part-time status, plus mileage reimbursement and paid time off; driver’s license and insured vehicle

Qualifications

• 1 year clinical experience or Clinical Affiliation in Home Care.
• Graduate of a Physical Therapy program is required.
• Bachelor’s Degree required; Master’s or Doctorate preferred.
• Current Pennsylvania Physical Therapy license; Provider BLS on hire.

Responsibilities

• Perform comprehensive evaluation and develop a plan-of-care based on findings.
• Plan and administer physical therapy treatments in the patient’s home.
• Provide patient and family education and guidance.
• Maintain accurate documentation of patient progress and outcomes.
