Job description

Overview

Provides support to new Physical Therapists as they begin their career. Provides physical therapy services to meet the needs of the patient/client and manages/coordinates the delivery of cost-effective multidisciplinary health care services for a caseload of patients/clients consistent with VNS Health philosophy, policy, goals, and objectives, and Standards of Physical Therapy. Attends seminars on applied learning topics, reflects on clinical experiences in supportive group settings, and shares insights with peers.

Participates in classroom learning activities covering critical thinking, evidence-based assessments, care planning, evaluation of care, and therapeutic interventions.

Shadows physical therapists to gain field experience and reviews experiences in group debriefs.

Provides physical therapy services to VNS Health home care patients through comprehensive assessments.

Develops and implements a plan of care aligned with VNS Health's goals and Rehabilitation policies and procedures.

Documents all services in accordance with government regulations and VNS Health policies, maintaining written and verbal communication with team members and management to ensure optimal patient care.

Provides therapeutic care following VNS Health standards, which may involve standing, stooping, sitting, crouching, bending, and stretching.

Transports and utilizes medical equipment and supplies, weighing approximately 25-30 lbs, to and from patient homes and care facilities.

Follows established work procedures and maintains productivity to meet organizational goals.

Travels to patient homes and facilities, which may include varied environments, using approved transportation options.

Assumes responsibility for personal professional growth through attendance at in-services, training, continued education, and journal readership.

Participates in special projects and performs other duties as assigned.
