Qualifications

• Bachelor’s Degree in Physical Therapy.
• Licensed Physical Therapist in Pennsylvania.
• Ability to travel to Home Care assignments.
• American Heart Association Basic Life Support (BLS) required.

Responsibilities

• Establishes and implements physical therapy plans of care to restore function and independence.
• Implements individualized treatment plans using evidence-based therapy techniques (exercises, manual therapy, mobility training).
• Provides PT treatment aligned with patient needs assessment and goals.
• Performs reassessment and modifies plans as indicated.
• Communicates patient progress and limitations to supervisor and care team.
