Allina Health is seeking aLicensed Physical Therapist to join our Home Care team in New Ulm, MN. This 0.5 FTE position involves 8-hour day shifts with occasional weekend work, serving clients across designated areas.

Requires a Bachelor’s degree in Physical Therapy, MN licensure upon hire, and 2–5 years in rehab or long-term care settings. A Master’s degree is preferred, with opportunities for growth within a not-for-profit health system.

Qualifications

• Bachelor's degree in Physical Therapy from an APTA accredited program.
• 2 to 5 years in a rehabilitation or long term care setting.
• Licensed Physical Therapist - MN State Board of Physical Therapy required if providing services in MN upon hire.

Responsibilities

• Provides Physical Therapy treatment to clients.
• Initiates assessment of client per diagnosis.
• Consults with client on goals and outcomes.
• Educates client and family on treatment plan including age-specific and cultural considerations.
• Identifies goals and intervention for each problem.
• Continuously assesses needs and progresses plan of care.
• Initiates referrals to other disciplines.
• Case management of clients as needed.
• Oversees plan of care and provides ongoing supervision.
• Makes interdisciplinary referrals.

Job Description

• Provides Physical Therapy treatment to clients.
• Initiates assessment of client per diagnosis.
• Consults with client on their goals and desired outcomes.
• Educates client, family members or others involved in care of treatment plan including age specific and cultural considerations.
• Identifies goals and intervention for each problem, impairment or functional limitations of client.
• Continuously assesses needs of client and functional deficits, progressing plan of care.
• Initiates appropriate referrals to other disciplines.
• Case management of clients as needed.
• Establishes and provides on‑going oversight of plan of care.
• Makes interdisciplinary referrals.
• Facilitates communication with disciplines involved including providers and payer sources.
• Manages caseload taking into consideration priorities, scheduling, urgency of client in and out of assigned geographical areas.
• Other duties as assigned.

Required Qualifications

• Bachelor's degree in Physical Therapy from an APTA accredited program
• 2 to 5 years in a rehabilitation or long term care setting

Preferred Qualifications

• Master's degree in Physical Therapy
• 0 to 2 years in a Home Care or acute care setting

Licenses / Certifications

• Licensed Physical Therapist - MN State Board of Physical Therapy required if providing services in MN upon hire
• BLS Tier 1 - Basic Life Support - Multisource required by completion of orientation
• Valid Driver’s License required upon hire - MN and WI residents must obtain a valid driver’s license in their state of residence within 60 days of employment

Physical Demands: Medium Work*: Lifting weight up to 32 lbs. occasionally (*Allina Safe Patient Moving Policy), up to 25 lbs. frequently.

Pay Range Pay Range: $42.92 to $59.38 per hour The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
