Key Responsibilities
  • Evaluate patients and develop individualized care plans tailored to functional goals
  • Provide evidence-based therapy interventions to promote strength, mobility, and independence
  • Collaborate with interdisciplinary care teams, including physicians, nurses, and fellow therapists
  • Document clinical services accurately and in compliance with regulatory standards
  • Monitor patient progress and adjust treatment plans as needed
  • Maintain timely and complete physician orders and care documentation
Qualifications
Required:
  • Degree from an accredited Physical Therapy program (DPT, MPT, or BSPT)
  • Current Physical Therapist (PT) license in the state of practice
  • CPR certification and compliance with state health and safety requirements
  • Excellent communication and time-management skills
Preferred:
  • New graduates encouraged to apply - comprehensive mentorship available
  • Experience in skilled nursing or long-term care settings
  • Familiarity with EMR documentation systems
  • Bilingual (Spanish, Tagalog, Korean, etc.) is a plus
