Qualifications

• Graduate of an accredited school of Physical Therapy.
• CPR/BLS within 90 days of hire.
• Licensed as a physical therapist in WI, IL, or both.
• Background checks as required.

Responsibilities

• Integrates subjective and objective data to establish a treatment plan.
• Utilizes exercise, physical agents, and manual techniques to address deficits.
• Maintains a thorough electronic documentation of services.
• Communicates with other disciplines and referring providers to enhance outcomes.
• Participates in PI and CQI processes; mentors and trains partners.
• Plans and conducts lectures and training programs for staff and community.
