Job summary

Retail Solutions Group is seeking a Physical Therapist in Rockford to support patients through prognosis discussions and care planning. You will work with families to align goals with care plans and participate in shift-change SBAR handoffs to ensure continuity.

You will also be responsible for maintaining supplies, validating blood products with a partner clinician, and monitoring leadership metrics to inform the care team.

Qualifications

• Clear communication translating medical jargon to families.

• ACLS certification and growing Telehealth interest.

• Experience in collaboration and healthcare space.

Responsibilities

• Sit with families through conversations about prognosis, code status, and goals of care.
• Hand off at shift change using SBAR, ensuring no gaps for the next clinician.
• Restock supply rooms and par levels so the next clinician won’t search for items.
• Verify blood products at the bedside with another clinician before transfusion.
• Track leadership metrics on your panel and surface trends to the care team weekly.
