Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Omaha, NE. This PRN position focuses on delivering quality therapy services, completing CMS-compliant documentation, and collaborating with residents and the interdisciplinary team.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and productivity guidelines.
• Document and bill therapy services in CMS-compliant NetHealth EMR.
• Evaluate residents, create treatment plans with short and long-term goals, and communicate findings to residents, families, and the interdisciplinary team.
• Complete compliant documentation as required by CMS in NetHealth EMR systems.
