Job summary

Avamere Sunnyside in Salem, OR, seeks a licensed Physical Therapist to provide and direct high quality patient care, including evaluation, treatment planning, goal setting, family education, and documentation.

You will supervise assistants and students, communicate with care teams and families, participate in quality assurance, and pursue ongoing education while maintaining licensure; this is a full‑time position with PRN shifts and a robust benefits package.

Qualifications

• Graduate of an approved Physical Therapy curriculum.
• Able to practice unencumbered.
• In good standing with regulatory agencies and licensing boards.
• Active CPR/BLS certification during employment.
• Working knowledge of Medicare and other payer sources.
• Full knowledge of residents’ rights.
• Exudes professionalism and the ability to multitask.

Responsibilities

• Provide physical therapy to patients according to licensure.
• Direct and supervise assistants, rehab aides, and students per guidelines.
• Follow safety procedures regarding personal protection and patient safety.
• Expand clinical knowledge through continuing education.
• Support and participate in department operations and development.
• Communicate patient status to the patient, family, caregivers, or other care team.
• Perform evaluations per MD orders and payer guidelines.
• Develop and provide treatment plans aligned with reimbursement.
• Educate staff and clinicians on benefits and outcomes.
• Complete timely documentation and meet productivity standards.
• Screen individuals who may benefit from therapy.
• Use professional judgment to ensure safety of patients and staff.
• Maintain licensure, certification, and credentialing.
• Submit billing, payroll, and expenses timely.
• Make referrals to other rehab team members.
