Job summary

The Sans Souci Rehabilitation & Nursing Center in New Rochelle, NY is seeking an experienced Physical Therapist to assess and treat patients in a long-term care setting. You will develop and implement individualized treatment plans under physician direction and supervise PT assistants.

Candidates must have a PT degree and license, strong documentation skills, and experience in geriatric care. Excellent patient service and confidentiality are essential to this role.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Overseeing and supervising Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Strong documentation writing.
• Evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.
• Excellent patient service skills and clinical excellence.
• Punctuality and professional behavior.
• Compliance with confidentiality, HIPAA and residents' rights.
• Experience with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals aligned to patient capacity and physician direction.
• Document and record patient condition; educate patients and families.
• Supervise and oversee Physical Therapy Assistants.
• Coordinate with other disciplines to deliver best patient-centered treatment.
