Job summary

Qualifications

• Must have graduated from an accredited Physical Therapy program.
• Must hold a valid NY professional license and registration.
• Oversee and supervise Physical Therapy Assistants.
• Experience in long-term/geriatric care preferred.
• Strong documentation and record-keeping skills.
• Experience with evaluations, progress notes, recertification and discharge documents.
• Collaborate with all disciplines to provide patient-centered care.
• Excellent patient service skills and clinical excellence.
• Punctuality and professional conduct at all times.
• Performs duties as assigned by Director of Rehabilitation.
• Adheres to confidentiality, HIPAA, and residents' rights.
• Experience with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct care.
• Establish and administer a tailored treatment program with goals.
• Document patient status and progress; educate patients and families.
• Coordinate care with physicians and other disciplines.
• Ensure compliance with regulatory and facility policies.
