FOX Rehabilitation is seeking licensed Physical Therapists for Georgetown, DE, offering one-on-one care to older adults in homes or senior living communities. You’ll deliver evaluations, develop plans, and provide evidence-based therapy with flexible scheduling and a supportive, interdisciplinary team.

The role emphasizes autonomy, meaningful patient outcomes, and opportunities to incorporate telehealth into your practice while maintaining high-quality in-person care.

Qualifications

• Degree from an accredited Physical Therapy program.
• Valid Physical Therapy license in the state(s) of practice.
• Current CPR certification.
• Basic computer and EMR proficiency.

Responsabilités

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, independent living, assisted living, memory care).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.

Physical Requirements

FOX clinicians must be able to meet the physical demands ofin‑homeandcommunity‑basedcare, including:

• Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly

• Performing CPR

• Providing moderate to maximumassistancewith patient transfers and bed mobility

• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities

• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model

• Wearing PPE whenrequired
