Job summary

Adelphi Nursing & Rehab Center is seeking a dedicated Physical Therapist (PT) for a full-time role in Adelphi, MD. The PT will provide direct patient care, develop and manage individualized treatment plans, and educate patients and families about rehabilitation strategies.

Qualifications

• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred

• Document and educate patients and families in a PT method.
• Collaborates with all disciplines to provide patient-centered care.

Duties and Responsibilities:

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.
• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.
• Collaborates with all disciplines to provide best patient centered treatment and care.
