Physical Therapist - Full Time Home Health

Position Details

We are seeking compassionate Home Health Physical Therapists (PT) to serve patients in North Carolina. Positions are available in Danbury, New Hope, Wilkesboro, Yadkinville, Greenville, Fayetteville, and Pinehurst. These are FULL-TIME roles.

Responsibilities

• Conduct home visits for clients in designated areas
• Perform diagnostic assessments including mobility, strength, balance, and safety evaluations
• Implement wound care and protective interventions using evidence-based methods
• Create and update individualized care plans involving manual therapy, gait, and balance training
• Collaborate with nursing staff on multidisciplinary care plans
• Educate clients and families on rehabilitative activities
• Document all interventions accurately using electronic tablets

Qualifications

• Active NC Physical Therapist license
• Graduate of an accredited PT program
• Ability to work independently and manage time effectively
• Strong interpersonal and organizational skills
• Computer proficiency; experience with EMR preferred
