Job summary

You will act as a co-leader within an interdisciplinary team, conducting evaluations, creating personalized plans, providing interventions, and ensuring precise documentation to support patient-centered care.

Qualifications

• DPT degree or foreign equivalent with licensure in state of practice.
• CPR/BLS certification.
• Preferred two years of experience.
