Split Between Two Vibrant Communities (West Dundee & Elgin). If you’re a Physical Therapist who loves connection, creativity, and helping older adults stay active and independent—this role is made for you.

We’re looking for our next employee‑owner to join a collaborative, upbeat rehab team supporting two welcoming senior‑living communities:

• West Dundee – Mondays, Wednesdays, Fridays
• Elgin – Tuesdays & Thursdays

Responsibilities

• Set meaningful goals and deliver purposeful, patient‑centered treatment
• Collaborate with an interdisciplinary team that actually listens
• Partner with and supervise PTAs who value your leadership
• Help your patients reclaim strength, function, and confidence
• Enjoy the stability of two consistent locations without the chaos of constant travel

Qualifications

• Graduate of an accredited Physical Therapy program
