Job summary

Live Well Homecare is seeking a licensed Physical Therapist to provide outpatient services focused on orthopedic, sports medicine, and neurologic patients. You will develop personalized treatment plans, perform evaluations, and guide interventions to improve mobility and independence.

Responsibilities include executing plans, coordinating with families and providers, and mentoring clinical staff. The role emphasizes patient-centered care in a supportive, community-based facility.

Qualifications

• Bachelor’s degree in Physical Therapy (Master’s preferred).
• Active Physical Therapist License.
• Experience is not required but preferred.
• Excellent communication, collaboration, and patient-centered care skills.

• Educate and collaborate with patients, families, and healthcare providers, making appropriate referrals.
