Job summary

Alton Memorial Hospital in Illinois is seeking a full-time Physical Therapist for an inpatient and/or outpatient setting with a set schedule. Hours include Monday–Thursday 9:00 AM–5:30 PM, Friday 7:00 AM–3:30 PM, plus weekend rotation.

You will work with a multidisciplinary team on a diverse outpatient caseload including orthopedic, neurological and general medicine patients, with a CEU budget and a sign-on bonus.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• License: Physical Therapist License required.
• Preferred: Master's Degree in Physical Therapy.

Responsibilities

• Provides skilled physical therapy services to patients, including evaluation and treatment.
• Establishes treatment plan and discharge planning based on functional status.
• Educates and collaborates with patients and families and other health care providers.

You’ll work alongside a dynamic, multidisciplinary team while managing a diverse outpatient caseload, including orthopedic, neurological, and general medicine patients.

Overview

Since 1937, Alton Memorial Hospital has cared for residents in Alton, Illinois, and the surrounding communities in a five-county area. Starting as a land gift to the community from the Smith family, Alton Memorial today is a full-service acute care hospital. Alton Memorial offers patients a variety of inpatient and outpatient services, including surgery services, medical imaging, interventional and diagnostic heart services, cancer care, rehabilitation, 24-hour emergency care, ambulance services and more.

Alton Memorial Hospital's Rehabilitation Services is called, 'The Human Motion Institute'. The Human Motion Institute's rehabilitation team includes Physical Therapists, Occupational Therapists, Speech Therapists, Athletic Trainers, Physical Therapist Assistants and Occupational Therapists Assistants.

Role Purpose

Provides skilled physical therapy services to patients including all aspects of evaluation and treatment.

Responsibilities

• Establishes treatment plan and provides treatment, considering discharge needs; expected discharge functional status; and appropriate equipment and supplies.
• Educates, collaborates with, and conferences with patients, families, and other health care providers; making recommendations for referrals to specialists as needed.
• Delegates to assistants, techs, and students through written and verbal communication including determining the skill level or competency required to provide interventions to best meet the needs of the patient.
• Completes documentation in accordance with departmental policies.
• Provides assessment, reassessment, and/or consultations for Physical Therapy Services.
