Job Description
The Physical Therapist assesses the need for, develops, and delivers physical therapy programs to facilitate rehabilitation. They identify the need for and make referrals to other disciplines to address the comprehensive needs of residents. They implement individualized therapy programs to maximize potential and educate residents and caregivers to promote the highest practical functional level.
Essential Duties and Responsibilities
- Supervise Physical Therapy Assistants, Physical Therapists with temporary licenses, students, and ancillary staff according to state licensure requirements and professional standards.
- Perform screens, evaluations, progress reports, treatment notes, and reevaluations promptly upon referral.
- Establish long-term and short-term goals based on evaluations in collaboration with residents, physicians, caregivers, and other involved disciplines.
- Obtain MD orders for evaluations, treatments, recertifications, and discharges.
- Ensure effective delivery and documentation of rehab services as per residents’ care plans.
- Participate in patient care and staffing meetings as assigned by DOR.
- Monitor and modify residents’ plans of care as needed.
- Promote safety for residents and staff throughout the facility.
- Prioritize residents’ needs to ensure excellent clinical care in a compassionate manner.
- Order, maintain, and report issues with supplies and equipment.
- Build professional relationships with residents, families, and staff for consistent communication.
- Assist with care and discharge planning to facilitate transitions.
- Support facility training as needed and be willing to travel within reasonable distances.
- Perform other duties as assigned.
Qualifications
- Graduate of an accredited Physical Therapy program (CAPTE approved).
- Valid Physical Therapy license in the practicing state(s).
- Minimum of 3 years’ experience in long-term care preferred.
Physical Demands
Must be able to stand, walk, kneel, use hands, talk, hear, reach, twist, bend, lift up to 50 pounds, and push/pull 200 pounds for short distances. Vision abilities include close, peripheral, and distance vision.
Skills
- Ability to interpret documents, write reports, and communicate effectively.
- Proficiency with Microsoft Office and familiarity with office equipment.
- Knowledge of state practice acts, Medicare, and third-party billing.
- Patience and understanding of elderly care, with a commitment to safety and infection control.
Additional Requirements
- Use of therapy equipment and flexible work schedule, including some weekends and holidays.
- Adherence to HIPAA and safety regulations.
