Ahava Therapy Services in Adamsville, TN, is seeking Physical Therapists for full-time, part-time, or PRN roles in a 140-bed skilled nursing facility offering short-term rehab, long-term care, and outpatient services. You will join an in-house therapy team under strong leadership to restore function and enhance quality of life.

Responsibilities include evaluating residents, creating individualized treatment plans, documenting progress, supervising assistants, and coordinating care with nursing

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy and current PT license in the state of practice.
• BLS/CPR certification (current or obtained during orientation).
• Strong communication, teamwork, and organizational skills.

Responsibilities

• Review resident information and evaluate its impact on assessment and treatment.
• Conduct and document functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with clear goals.
• Maintain timely, accurate records and comply with guidelines.
• Educate residents and families on home exercise and therapy continuation.
• Collaborate with nursing staff and families to maximize functional abilities.
• Participate in rehabilitation team conferences and facility meetings.

About the Facility

Adamsville Healthcare & Rehab Center is a 140-bed skilled nursing facility offering short-term rehab, long-term care, and outpatient therapy services.
The facility also includes a 46-bed secure memory care unit with a state-of-the-art sensory room.

Responsibilities

• Review resident information and evaluate its impact on assessment and treatment.

• Conduct and document thorough functional assessments using standardized procedures.

• Develop and implement individualized treatment plans with achievable goals.

• Comply with evaluation, treatment, and documentation guidelines; maintain timely, accurate records.

• Instruct and supervise therapy assistants as needed.

• Enter treatment and progress notes for each session.

• Revise treatment plans as needed throughout the course of care.

• Document treatment in clear, concise discharge summaries.

• Orient residents to services and treatment procedures appropriate to their needs.

• Collaborate with nursing staff, residents, and families to maximize functional abilities.

• Educate residents and caregivers on the continuation of home exercise and therapy.

• Assist with referrals for additional services as needed and maintain knowledge of community resources.

• Participate in rehabilitation team conferences and other facility meetings.

• Access continuing education opportunities to maintain clinical competency.

• Participate in quality improvement initiatives.

• Follow regulatory, professional, and facility standards.

• Perform other duties as assigned to provide quality care.

• Understand payer reimbursement and allocate therapy resources efficiently.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy

• Current PT license in the state of practice

• BLS/CPR certification (current or obtained during orientation)
