Job summary

Ahava Therapy Services seeks a Full-Time Physical Therapist to join our in-house rehab team at Henry County Health and Rehabilitation Center in Paris, TN. You will assess residents, develop and implement individualized treatment plans, and collaborate with nursing staff to maximize functional outcomes.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy.
• Current PT license in the state of practice.
• BLS/CPR certification (current or obtained during orientation).
• Strong communication, teamwork, and organizational skills.

Responsibilities

• Review resident information and evaluate its impact on assessment and treatment.
• Conduct functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable goals.
• Comply with evaluation, treatment, and documentation guidelines; maintain timely records.
• Instruct and supervise therapy assistants as needed.
• Enter treatment and progress notes for each session.
• Revise treatment plans as needed throughout care.
• Document discharge summaries clearly and concisely.
• Orient residents to services and procedures relevant to their needs.
• Collaborate with nursing staff, residents, and families to maximize functional abilities.
• Educate residents and caregivers on home exercise and therapy continuation.
• Assist with referrals for additional services as needed and know community resources.
• Participate in rehab team conferences and facility meetings.
• Access continuing education opportunities to maintain competency.
• Follow regulatory, professional, and facility standards.
• Perform other duties as assigned to provide quality care.
• Understand payer reimbursement and allocate therapy resources efficiently.
• Support Ahava’s mission and project a professional image.
• Maintain expected productivity levels.

About the Facility

Ahava Therapy Services is seeking a Full-Time PT to join our in-house rehab team at Henry County Health and Rehabilitation Center, a 136-bed skilled nursing facility providing short-term rehabilitation, long-term care, and outpatient services.
