Job summary

South Pacific Rehab Services is hiring a licensed Physical Therapist for a Full-Time position in a Skilled Nursing Facility/Long-Term Care setting in California. We offer a supportive culture, flexible schedules, comprehensive health benefits, and ongoing professional development.

You will evaluate and treat patients, develop personalized care plans, and collaborate with the care team while ensuring regulatory documentation and up-to-date physician orders are maintained.

Qualifications

• Degree from an APTA-accredited Physical Therapy program (BS, MS, or DPT).
• Current license as Physical Therapist in the state of practice.
• New grads welcome; mentorship available.

Responsibilities

• Evaluate and treat patients with mobility and functional impairments.
• Create and implement personalized care plans.
• Collaborate with team to ensure quality outcomes and regulatory compliance.
• Maintain documentation to meet all regulatory requirements.
• Ensure physician orders are current and properly obtained.
