Renewal Rehab is seeking a licensed Physical Therapist for a full-time role in a Skilled Nursing Facility / Long-Term Care setting. You will evaluate and treat patients, develop personalized care plans, and collaborate with a multidisciplinary team to achieve high-quality outcomes.

Qualifications

• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Ensure all physician orders are obtained and up to date.
