Responsibilities

• Provide comprehensive physical therapy evaluations based on MD’s orders
• Develop treatment plans with goals, frequency, and modalities
• Ensure MD orders are obtained for evaluations, treatments, re-certifications, and discharges
• Administer treatments using modalities and modify as needed, adhering to precautions
• Complete all required documentation

Qualifications

• Graduate of an accredited university with recognized degree (B.S., Doctorate, MA, MS) in Physical Therapy
• Current license/registration as a Physical Therapist in Ohio
