Purpose and Overall DutiesThe Physical Therapist (PT) evaluates, plans, and implements individualized physical therapy programs for patients in a long-term care or related setting. The role focuses on improving mobility, strength, balance, and functional independence while reducing pain and preventing further injury.II. Essential Job FunctionsA. Patient Evaluation and Treatment PlanningEvaluate patients’ physical condition, mobility, strength, balance, and functional abilities using standardized assessments and clinical judgmentDevelop individualized treatment plans based on patient needs, physician orders, and regulatory requirementsEstablish measurable goals and modify treatment plans as necessary based on patient progressB. Patient Care / InterventionImplement therapeutic interventions to improve mobility, strength, balance, coordination, and enduranceProvide gait training, transfer training, and therapeutic exercise programsUtilize manual therapy techniques and therapeutic modalities as appropriateEducate patients and caregivers on exercises, safety precautions, and home programsRecommend and train patients in the use of assistive devices (e.g., walkers, canes)C.
