Physicians Choice Home Health in the South Bay area is hiring a Physical Therapist to join our Home Health team, delivering in-home care to help patients regain independence. You will oversee PTAs and coordinate with nursing and therapy staff to support positive outcomes.

You will assess patients, develop plans of care, educate families, and contribute to discharge planning, working independently in a home-health setting with flexible hours.

Qualifications

• Graduate of an accredited Physical Therapy program (Bachelor’s, Master’s, or DPT).
• Active California Physical Therapist license.
• CPR/BLS certification is required or current.
• Strong communication and organizational skills.
• Reliable transportation and willingness to travel to patients’ homes.
• Comfort working independently in home health settings.

Physicians Choice Home Health is hiring a full-time or part-time/per diem Physical Therapist (PT) to join our Home Health team, serving patients in the South Bay of Los Angeles. In this role, you will provide high-quality physical therapy services to patients in the comfort of their own homes, helping them regain independence and improve their daily functioning. You will oversee a supporting Physical Therapy Assistant and a team of dedicated healthcare professionals who are committed to delivering exceptional patient-centered care.

What You'll Do

• Provide in-home physical therapy evaluations and treatments
• Assess patients’ strength, mobility, balance, and ability to function safely at home
• Create and update individualized plans of care that support independence and recovery
• Help patients regain mobility and confidence in their home environment
• Educate patients and caregivers on exercises, safety, and fall prevention
• Document visits and evaluations in WellSky/Kinnser in a timely manner
• Collaborate with nurses, therapists, and the broader care team to support patient outcomes
• Supervise Physical Therapy Assistants (PTAs) and support their clinical development
• Participate in care planning, case conferences, and discharge planning

Requirements

• Graduate of an accredited Physical Therapy program (Bachelor’s, Master’s, or DPT)
• Active California Physical Therapist license
• CPR/BLS certification
• Strong communication and organizational skills
• Reliable transportation and willingness to travel to patients’ homes
• Comfort working independently in a home health setting
• Home health experience preferred
• Experience charting in Kinnser/Wellsky preferred, but not required
