Job summary

Physicians Choice Home Health, part of Obran Cooperative, is seeking an Occupational Therapist to deliver in-home OT services in Southern California. You will supervise a COTA and collaborate with a multidisciplinary team to restore independence and daily functioning.

The role involves travel to patient homes, completing evaluations, creating treatment plans, and educating patients and caregivers. Certifications and CA license are required.

Qualifications

• Graduate of an accredited OT program (Master’s or Doctorate).
• Active California OT license.
• Current CPR/BLS certification.
• Reliable transportation and willingness to travel to patient homes.
• Home Healthcare experience.
• Experience documenting in Kinnser/WellSky EMR preferred.

Responsibilities

• Provide in-home OT evaluations and treatments per physician’s plan of care.
• Assess patients’ strength, mobility, and home safety.
• Develop and update individualized treatment plans.
• Assist patients to improve independence with daily activities.
• Educate patients and caregivers on exercises and adaptive techniques.
• Document evaluations and progress in EMR in a timely manner.
• Communicate progress and changes to the care team and physician.
• Supervise COTAs in accordance with state guidelines.
• Participate in care planning, conferences, and discharge planning.

Job description

Physicians Choice Home Health is hiring a full-time or part-time/per diem Occupational Therapist to join our Home Healthcare Team and see patients in Southern California's South Bay Area. In this role, you will provide high-quality occupational therapy services to patients in the comfort of their own homes, helping them regain independence and improve their daily functioning. You will supervise a Certified Occupational Therapy Assistant (COTA) and work with a team of dedicated healthcare professionals who are committed to delivering exceptional patient-centered care.

What You'll Do

• Provide in-home occupational therapy evaluations and treatments according to the physician’s plan of care
• Assess patients’ strength, coordination, mobility, and ability to function safely at home
• Develop and update individualized treatment plans based on patient goals and progress
• Help patients improve independence with daily activities such as dressing, bathing, mobility, and home safety
• Educate patients and caregivers on exercises, adaptive techniques, and safe movement
• Document evaluations, visits, and progress in WellSky/Kinnser in a timely manner
• Communicate patient progress, concerns, and changes in condition to the care team and physician
• Supervise COTAs in accordance with state guidelines
• Participate in care planning, case conferences, and discharge planning

• Graduate of an accredited Occupational Therapy program (Master’s or Doctorate in Occupational Therapy)
• Active California Occupational Therapist license
• Current CPR/BLS certification
• Reliable transportation and willingness to travel to patient homes
• Home Healthcare experience

• $40-120/hour, depending on task/visit type
