Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA, or MS in Physical Therapy recognized by APTA
• Current license and/or registration as a Physical Therapist in-state

Responsibilities

1. Conduct comprehensive physical therapy evaluations based on MD’s orders
2. Develop treatment plans with goals, frequency, duration, and modalities
3. Ensure MD orders are obtained for evaluations, treatments, re-certifications, and discharges
4. Provide treatment utilizing modalities and modify as needed while following precautions
5. Complete all required documentation
