HealthPro Heritage in Rocky Hill, CT is seeking a Float Physical Therapist for our inpatient and outpatient SNF settings. This full-time role offers a stable weekly schedule in the same building with minimal travel between facilities.

You will evaluate and treat a diverse caseload, collaborate with an interdisciplinary team, and participate in professional development opportunities within a supportive, growth-oriented environment.

Qualifications

• Degree in Physical Therapy from an accredited institution.
• Valid state licensure as a Physical Therapist, or license eligible.
• Strong clinical assessment and diagnostic skills with patient-centered approach.
• Excellent communication and interpersonal skills.

Responsibilities

• Patient Assessment: Evaluate patients' physical conditions, mobility, strength, and flexibility.
• Treatment and Education: Provide exercises, manual therapy, and education on home routines and lifestyle changes.
• Documentation: Maintain accurate records of evaluations, progress, and treatment plans.
• Collaboration: Work with an interdisciplinary team to ensure effective patient care.

• Collaboration: Work with healthcare providers to ensure effective patient care and participate in multidisciplinary team meetings as needed.

Qualifications

• Education: Degree in Physical Therapy from an accredited institution.

• Skills: Strong clinical assessment and diagnostic skills. Proficient in developing and implementing effective treatment plans. Excellent communication and interpersonal skills with a patient-centered approach.
• Physical Requirements: Ability to lift and assist patients as needed. Must be able to stand for extended periods and perform physical tasks related to therapy.
