Job summary

Compassionate Care Home Health is seeking a Physical Therapist to provide patient care in Mariposa and surrounding areas. The role offers a hybrid model with Pay-Per-Visit and hourly compensation, negotiated based on experience and availability.

You will assess patients, develop treatment plans, educate families, and coordinate with physicians. Experience in home health and CPR certification are preferred, with a valid CA PT license required.

Qualifications

• Degree from a masters or doctoral program in physical therapy approved by an accredited organization.
• Licensed to practice as a physical therapist within the state, if applicable.
• One year of appropriate experience as a physical therapist. Community/home health experience is preferred.
• Demonstrates good verbal and written communication and organization skills.
• Possesses and maintains current CPR certification.
• Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

Responsibilities

• Provides physical therapy services to patients according to a written physician's plan of care and state PT Act.
• Evaluates muscle strength, mobility, gait, range of motion for rehab potential.
• Directs physical therapy treatment.
• Instructs patients and families in use and care of therapeutic appliances.
• Initiates PT program and educates staff/family in therapy phases.
• Participation in discharge planning and progress summaries.
