Riverview Estates is seeking a Physical Therapist to join their team in New Jersey. You will support treatment plans, educate residents and care staff, and collaborate with a multidisciplinary rehab team to ensure optimal outcomes.

The role emphasizes detailed documentation, remote monitoring, and a patient‑centered approach in a community‑driven environment with opportunities for growth and advancement.

Qualifications

• Assist the Physical Therapist in carrying out treatment plans and collect objective data.
• Support treatment plans with exercises, modalities and mobility training.
• Monitor residents and report changes to the supervising PT.
• Maintain accurate documentation of resident progress and interventions.
• Educate residents and care staff on mobility techniques and safe exercises.
• Coordinate with rehab staff and participate in RTM remote monitoring.

Responsibilities

• Assist in implementing treatment plans and gather data for screenings.
• Support treatment plans with exercises and mobility training.
• Monitor residents' performance and report changes.
• Maintain accurate documentation in EMRs (Net Health/PCC) and PoS care.
• Educate residents and families on safe techniques; perform RTM check-ins.
• Collaborate with interdisciplinary team and rehab staff.
