Qualifications

• Degree in Physical Therapy required (Bachelor's or Master's).
• Minimum 2 years of experience as a Physical Therapist.
• Experience in outpatient, home health or PACE setting preferred.
• CPR certification or willingness to obtain.
• Proficiency with EMR systems.
• Reliable transportation and a valid driver's license.

Responsibilities

• Assess strength, balance, and mobility; develop personalized, evidence-based treatment plans.
• Lead engaging group exercise programs and track progress.
• Collaborate with the Interdisciplinary Team for coordinated care.
• Document care and supervise program aides as needed.
