Job summary

Abbotsford Health Care Center in Abbotsford, WI, is recruiting a Physical Therapist to join our compassionate team. The role focuses on evaluating, treating, and documenting resident care to restore independence where possible, within interdisciplinary teams and facility policies.

You will contribute to restorative nursing programs and participate in care conferences, with a goal of maintaining residents' highest possible level of function in a supportive environment.

Qualifications

• Excellent verbal and written communication and organizational skills.
• Ability to read, analyze, and interpret common medical information.
• Ability to present information to supervisors.
• MS in Occupational Therapy and NBCOT certification required.

Responsibilities

• Evaluate and document resident assessments according to facility policies and procedures.
• Provide appropriate treatment to maximize residents' function.
• Recommend adaptive equipment and preventative measures.
• Educate residents and caregivers to support ongoing function.
• Maintain accurate resident reports for regulatory and payer requirements.
• Participate in care conferences and rehabilitation planning.
• Complete documentation for evaluations, treatment plans, MDS, and care plans in a timely manner.
