Job summary

Alliance Health at Baldwinville is seeking a Physical Therapist to join our Rehab team in Skilled/Long Term Care and Outpatient Clinic.

Responsibilities include evaluating and screening all patient referrals to the Rehabilitation Department for Physical Therapy, interpreting evaluations, and consulting with the rehab team to plan goals. Work is on-site with a day shift and flexible hours, collaborating with an Interdisciplinary Team to develop the resident care plan.

Qualifications

• Licensed Physical Therapist with current license.
• CPR certification preferred.
• Bachelor’s degree in Physical Therapy.
• Experience in long-term care or outpatient rehab preferred.
