Job summary

Responsibilities include evaluations, treatment planning, gait/balance training, fall prevention, progress monitoring, and interdisciplinary collaboration. Licensing in CA is required.

Qualifications

• Graduate of Physical Therapy from an accredited school.
• Active California PT license.

Responsibilities

• Perform evaluations and develop individualized treatment plans.
• Provide therapeutic exercises, gait training, balance training, and mobility interventions.
• Implement fall prevention and safety strategies.
• Monitor patient progress and adjust care plans as needed.
• Complete timely and accurate documentation per regulatory guidelines.
• Collaborate with interdisciplinary team members.
• Participate in care planning and discharge planning.
• Educate residents and caregivers on exercises and safety techniques.
