Job Title: Physical Therapist PACE

Job Summary

The Physical Therapist evaluates and reassesses participants to assess mobility and other functional abilities. Provides and recommends therapeutic exercises, gait training, ambulation, etc. Plans and develops group exercise programs for maintenance therapy. Responsible for the overall Physical Therapy program at Florida PACE Centers.

Essential Job Functions

  • Evaluates, identifies, and provides therapeutic interventions as indicated in the care plan
  • Develops comprehensive treatment plans to maximize participant potential
  • Reviews participant status regularly and adjusts treatment as needed
  • Completes assessments and discharge summaries
  • Plans maintenance, exercise, and therapy programs
  • Supervises program aides and staff implementing therapy
  • Provides in-service education to staff, students, and the community
  • Documents treatments and maintains records
  • Participates in multidisciplinary team meetings and case conferences

Education

  • Graduate from an accredited Physical Therapy program approved by relevant organizations

Experience

  • At least one year of physical therapy experience working with frail or elderly populations

Licenses/Certifications

  • Florida State License as a Physical Therapist

Abilities Required

  • Knowledge of safety issues in treatment, including body mechanics and precautions
  • Effective communication and human relations skills
  • Bilingual in English/Spanish required; Creole a plus

  • Normal working conditions with adequate lighting and ventilation
  • Routine exposure to blood, body fluids, tissues

  • Adhere to residents’ rights and demonstrate professionalism
  • Maintain confidentiality and security of medical information
  • Participate in departmental activities and maintain a safe environment
