Exeter Hospital, part of Beth Israel Lahey Health, is seeking a Full Time Physical Therapist for its Out-Patient Rehabilitation Department in Exeter, NH. A $10,000 signing bonus is offered.

Qualifications

• Bachelor's degree; graduate of accredited program.
• Licensed Physical Therapist in New Hampshire (NH).
• CPR certified within 90 days of hire
• Preferred Lymphedema and Oncology experience

Responsibilities

• Selects; administers and interprets clinically appropriate assessments.
• Effectively integrates educational; medical; social and psychological factors when interpreting evaluative measures and determining the appropriate treatment diagnoses.
• Develops and implements individual plans of care which promote development and/or restoration of skills for daily living.
• Collaborates with other healthcare, contract, and school providers to coordinate services and maximize outcomes.
• Evaluates treatment outcomes at various stages and modifies the plan of care accordingly.
• Designs home exercise programs to facilitate carry over and promote independence; provides instruction and supervision in implementation of home programs.
• Maintains HIPAA-compliant records and ensures patient confidentiality.
• Maintains safe and therapeutic environment per hospital standards; mentors assistants and students.

Major Responsibilities

• Selects; administers and interprets clinically appropriate assessments.
• Effectively integrates educational; medical; social and psychological factors when interpreting evaluative measures and determining the appropriate treatment diagnosis/diagnoses.
• Develops and implements individual plans of care which promote development and/or restoration of skills required for successful participation in community activities and activities of daily living.
• Collaborates with other healthcare; contract; and school providers to coordinate services and maximize outcomes.
• Evaluates treatment outcomes at various stages and modifies the plan of care to reflect any changes in the patient's performance or condition.
• Designs home exercise programs to facilitate carry over and promote independence. Provides instruction and supervision in the implementation of home programs.
• Complies with standards for medical; and/or educational; records management (e.g. insurance authorization; consents; physician's referral; documentation of treatment notes) and maintains patient/client confidentiality in accordance with HIPAA guidelines.
• Complies with professional and licensure standards on code of ethics.
• Maintains a safe and therapeutic environment for patient and family in accordance with hospital standards including assurance and support of patient's rights and responsibilities.
• Orients; instructs and directs work activities of assistants; aides and/or students in acute; outpatient and contract settings.

Pay Range

$34.53 - $56.63

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
