Position Summary

This position is responsible for therapy evaluations, treatment planning, implementation, and discharge planning for clients in the home setting, in accordance with established facility/agency policies, while maintaining high-quality client care. As a member of the multi-disciplinary team, the therapist communicates client-related issues to appropriate team members or staff and participates in data collection and evaluation to assess the appropriateness of care at home. The Physical Therapist also oversees clients treated by home health aides and may orient staff and students. The therapist works with clients and their families in the home setting without direct supervision.


Position Specific Responsibilities

  1. Perform comprehensive assessments to determine therapy needs, develop care plans, coordinate with other disciplines, and provide direct patient care under physician orders.
  2. Complete all required documentation accurately and promptly.
  3. Promote and practice safety for patients, families, and staff according to policies.
  4. Participate in educational activities to improve care quality and seek professional growth.
  5. Display professionalism in communication and interactions with internal and external stakeholders.
  6. Maintain productivity standards as per agency guidelines.
  7. Comply with all relevant laws, regulations, and policies, including HIPAA and infection control protocols.
  8. Maintain confidentiality of all client and company information at all times.


Education and/or Experience

  • Required: Bachelor's Degree in Physical Therapy from an accredited institution.
  • Preferred: 1 year of therapy experience with adult physical disabilities.


Skills

  • Mathematical Skills: Ability to perform basic calculations, including rate, ratio, and percentage.
  • Language Skills: Ability to read, interpret, and communicate technical and professional information effectively in English.
  • Other Skills: Ability to work collaboratively, prioritize tasks, adapt to changes, and demonstrate organizational skills.
  • Knowledge of rehabilitation techniques applicable to diverse age groups, from adolescents to geriatrics.


Certifications, Licenses, and Registrations

  • Required upon hire: Washington Physical Therapist License.
  • Within 30 days: BLS certification (American Heart Association), NPI number.


Physical Demands and Work Environment

The role requires standing, walking, manual dexterity, and the ability to lift up to 50 pounds. Visual acuity and exposure to typical healthcare environment conditions are expected. Adaptations can be made for disabilities. This overview may vary by department.
