Qualifications

• Current State Physical Therapy License required.

Responsibilities

• Provide comprehensive physical therapy evaluations and treatments for residents in a skilled nursing setting.
• Develop and implement individualized treatment plans based on resident needs and physician recommendations.
• Administer skilled physical therapy interventions to improve mobility, strength, balance, and overall function.
• Monitor resident progress and adjust treatment plans as appropriate.
• Collaborate with interdisciplinary team members to ensure coordinated and effective care.
• Maintain accurate and timely clinical documentation in accordance with regulatory standards.
• Ensure the highest degree of quality care and resident safety is maintained at all times.
