Sobe Innovative Rehab in Sandy Springs, GA is seeking a licensed Physical Therapist (DPT/PT/RPT) to provide hospice therapy in patients' homes on a PRN basis.

You will evaluate residents, develop treatment plans, collaborate with physicians and families, and maintain EMR documentation while enjoying flexible assignments and competitive compensation.

Qualifications

• Licensed Physical Therapist in GA (PT) with active license.
• Experience or interest in Home Health/Hospice care.

• Evaluate and treat residents in an assisted living community and or Home setting
• Develop and implement individualized treatment plans
• Collaborate with physicians, families, and the interdisciplinary care team
• Maintain accurate, timely documentation in the EMR

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
• Active GA Physical Therapist (PT) license
• New graduates and experienced therapists are encouraged to apply
