Job summary

Talented Medical Solutions seeks a compassionate Physical Therapist to provide patient-centered rehabilitation across acute and post-acute settings. You will evaluate patients, develop individualized plans, and implement interventions to restore mobility and reduce pain.

Collaborating with physicians and the care team, you will educate patients and families, document progress, and ensure safety and compliance while delivering evidence-based care.

Qualifications

• Active PT license in the state of practice.
• BLS/CPR certification preferred or required per facility policy.
• DPT degree or equivalent as required by state regulations.
• Experience in acute care, outpatient, inpatient rehab, SNF, home health, or specialty settings preferred.
• Strong clinical assessment, communication, documentation, and patient education skills.
• Ability to work independently and collaboratively within a healthcare team.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on functional needs and referrals.

Requirements

• Active Physical Therapist (PT) license in the state of practice.
• Current Basic Life Support (BLS/CPR) certification preferred or required based on facility policy.
• Doctor of Physical Therapy (DPT) degree or equivalent educational requirements as required by state regulations.
• Previous experience in acute care, outpatient, inpatient rehabilitation, skilled nursing, home health, or specialty-specific settings preferred.
• Strong clinical assessment, communication, documentation, and patient education skills.
• Ability to work independently and collaboratively within an interdisciplinary healthcare team.
