Job summary

FOX Rehabilitation seeks licensed Physical Therapists to deliver in‑home and senior living care—including telehealth—to older adults in Cypress and surrounding communities, using our Geriatric House Calls model.

You will perform evaluations, develop plans of care, deliver evidence-based treatment, and document in FOX’s EMR. The role offers full-time, part-time, and PRN options with benefits and potential H1B sponsorship.

Qualifications

• Valid Physical Therapy license in the state(s) of practice or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in homes, senior living communities, and memory care settings
• Perform evaluations, develop plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to support wellness, mobility, and functional independence
• Document effectively within FOX’s EMR system per regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth or virtual care when appropriate to extend access

Physical Therapist – Cypress, TX

Location: Cypress, TX

We are seeking licensed Physical Therapists to deliver in‑home and senior living care—including telehealth—to older adults in Cypress and surrounding communities, using our Geriatric House Calls model.

Responsibilities

• Provide one‑on‑one physical therapy to older adults in private residences, senior living communities, independent living, assisted living, and memory care settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence‑based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth or virtual care when appropriate to extend access and flexibility.

Employment Types Available

• Full‑time Physical Therapist – In‑Home & Senior Living Care (salaried with benefits)

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Physical Therapy program

Physical Requirements

• Ability to exert medium physical effort, including lifting 25–50 lbs occasionally and 10–25 lbs frequently
• Perform CPR
• Provide moderate to maximum assistance with patient transfers and bed mobility
• Reposition patients, stand/walk for extended periods, climb stairs, and perform therapeutic activities
• Drive to patient homes and senior living communities as part of the Geriatric House Calls model
• Wear PPE when required
