Job summary

Easterseals seeks a School-Based Physical Therapist Coordinator to lead and support PT services in a school setting. This licensed professional will develop evaluation protocols, guide treatment planning, and align services with student goals.

The role requires collaboration with school teams, strong organizational skills, and a passion for helping children succeed through coordinated therapy and leadership in scheduling and service delivery.

Qualifications

• Active Illinois Physical Therapist license.
• Minimum of 2 years of experience in a school-based physical therapy setting.
• Current Child CPR and First Aid certification (or willingness to obtain).
• Prior supervisory or leadership experience preferred.
• Strong organizational, communication, and collaboration skills.

Responsibilities

• Conduct evaluations and update progress notes.
• Develop individualized, goal-driven treatment plans and reassess students.
• Collaborate with interdisciplinary teams to optimize scheduling and outcomes.
• Oversee scheduling for evaluations, referrals, and screenings.

What You’ll Do

• Conduct initial and ongoing evaluations, maintaining accurate documentation and progress notes.
• Develop individualized, goal-driven treatment plans and reassess students according to established timelines.
• Collaborate with interdisciplinary teams to optimize scheduling and maximize therapeutic outcomes.
• Oversee scheduling for evaluations, referrals, and screenings.

What You Bring

• Active Illinois Physical Therapist license.
• Minimum of 2 years of experience in a school-based physical therapy setting.
