Job summary

Comfort Home Health & Hospice is seeking a Licensed Physical Therapist to join our Nevada home health team. You will design individualized care plans and provide hands-on therapy in patients’ homes, collaborating with a supportive team to ensure quality outcomes.

Responsibilities include evaluating functional status, mobility, and home environments, educating patients on therapy techniques, and maintaining thorough documentation to inform physician progress.

Qualifications

• Nevada-licensed Physical Therapist or eligible to practice in NV.
• Current CPR certification.
• Minimum 2 years of home health experience preferred.

Requirements
