Responsibilities

• Prepares for evaluation including patient history and systems review.
• Selects tests, measures, and practices according to evaluation results.
• Forms diagnosis, goals, prognosis and plan of care.
• Establishes a person-centered plan of care and collaborates with the interdisciplinary team.
• Assesses and updates plan of care as needed.
• Initiates discharge planning and educates patients.
• Delivers interventions including therapeutic exercise and manual therapy.
• Records data for billing and compliance; adheres to regulations.
• Supervises other personnel.
• Lifts 50 lbs, pushes/pulls 50 lbs, stands 1 hour daily.
