Description

Focused Post Acute Care Partners (FPACP) is a dynamic company committed to excellence in long-term care. We value our culture and recognize outstanding care and kindness through our ROCKStar program, which involves team members, residents, families, volunteers, and partners. We are seeking a Licensed Physical Therapist to join our team.

Job Title: Physical Therapist

Department: Therapy

Reports to: Rehab Director, EDO

Summary: Evaluate patients, develop and implement physical therapy treatment plans, administer treatments, instruct and motivate patients, and supervise Physical Therapy Assistants if assigned.

Qualifications: Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited institution. Current state license/registration as a Physical Therapist.

Essential Duties:

  • Establish, implement, and evaluate treatment programs and goals in collaboration with physicians.
  • Provide direct patient care using various modalities and therapies.
  • Ensure ethical and professional standards are maintained.
  • Coordinate with other healthcare providers and participate in interdisciplinary team meetings.
  • Maintain proper documentation and coding for services.
  • Adhere to infection control standards and safety protocols.
  • Participate in staff meetings, in-services, and ongoing education.
  • Supervise Physical Therapy Assistants.

Physical Demands: Ability to lift up to 50 pounds, perform transfers, reposition patients, and perform repetitive upper extremity movements.

Work Environment: Exposure to unsanitary conditions and high-crime areas may occur. Reasonable accommodations will be provided as needed.
